What is IMM 5604?

IMM 5604 is a one-page IRCC declaration made by a parent or guardian who is not moving to Canada with their child. By signing it, that parent states that they have no objection to the child immigrating with the other parent or guardian, and that they understand they may be separated from the child permanently.

It is typically requested when only one parent is part of the immigration application, for example after a separation or divorce. A former spouse or former common-law partner signs it too. IRCC asks for the completed form together with a photocopy of the signing parent's identity document.

Who signs IMM 5604, and in front of whom

Only the parent or guardian who stays behind signs it. The parent who immigrates with the child does not sign this form; their name simply appears in the middle of the page.

The form itself says it must be witnessed by a notary public, and the bottom section is completed by that notary: their title and name, their stamp, their signature and the date. With us, the signing parent joins a video call from wherever they live, shows their passport on camera and signs on screen, and the notary public completes the witness section.

One form for each child

IRCC states it plainly at the top of the form: one IMM 5604 per child. A family with two children needs two signed declarations, even though every other line is the same.

You do not have to type everything twice. Add up to four children in the form above and we prepare a separate official page for each of them, with your name, the parent who travels and your phone number repeated on every page. Each page is one document on the booking, so a declaration for two children is booked as one document plus one additional document.

Mistakes that get IMM 5604 returned

  1. One form for several children. Listing two children on one page is the most common reason the declaration has to be redone.
  2. A signature that was not witnessed. Signing at home and having the page stamped later does not meet the form's own instruction. The notary has to see you sign.
  3. Names that do not match the passports. A shortened name or a missing middle name on the child's line can delay the application.
  4. Forgetting the copy of your ID. IRCC wants a photocopy of a valid identity document of the signing parent with the form. The photo ID you upload when booking is a good copy to keep for it.
  5. The witness section filled in by the parent. The lines for the witness's name, stamp and date belong to the notary public. Leave them alone.

Signing IMM 5604 with a notary public by video

When you press Book and Sign, the finished pages are attached to your booking, so there is nothing to scan or upload. Choose a time, add a photo of your passport and pay. On the call, the notary public checks your passport, you confirm each page is correct and sign it on screen, and the notary completes the witness section. The commissioned declaration arrives by email, ready to go into the application together with the copy of your ID.

Because the call is online, it does not matter where the non-accompanying parent lives: in Ontario, elsewhere in Canada or in another country.
