Affidavit of Identity — Online Notarization in Ontario
A sworn statement confirming that you are the person described in a specific document or photograph, notarized over a secure video call.
Institutions sometimes need more than a photo ID to be satisfied that you are who a particular document or photograph says you are — often when a name has changed slightly across records, or an older photo no longer resembles you closely. An Affidavit of Identity fills that gap with a sworn statement.
The affidavit must reach us unsigned. You sign live during your video appointment while the commissioner verifies your photo ID and witnesses the signature, then applies the seal.
Commissioned in accordance with O. Reg. 431/20. We do not advise on whether a particular institution will accept this affidavit for their specific purpose — check with them directly if you are unsure.
