What makes the 1155E different from the 1157E
Both forms come from the Ontario Ministry of Finance and both ask ServiceOntario not to charge retail sales tax on a used vehicle. The 1157E covers one situation only: a gift between listed family members. The 1155E is the general one. It cites several clauses of section 4.2(4) of the Retail Sales Tax Act and asks you to explain, in your own words, why the transfer happened.
In practice it is used, for example, when a vehicle moves from one spouse to the other after a marriage breakdown, or between a corporation and its shareholder. For corporate transfers the form itself warns that an information addendum from ServiceOntario must be attached. Which clause fits your case is a question for ServiceOntario or the Ministry; we do not advise on it.
The five numbered items
| Item | What it says | What you give us |
|---|---|---|
| 1 | You know the facts personally | Nothing, it is printed |
| 2 | The recipient now owns the vehicle | Nothing, it is printed |
| 3 | Year, make, model and VIN | Copied from the vehicle permit |
| 4 | Who acquired it, when, from whom, and as a result of what | The date and a short reason, such as "Marriage breakdown" |
| 5 | The detailed reason for the transfer | Up to three lines in your own words |
The names and the municipality lines come from the first two sections of our form, so you type each name once. Pick your city and the county or regional municipality is printed for you, or left empty for single-tier cities such as Toronto or Hamilton.
Writing the reason in item 5
Keep it factual and specific: what happened, the document it rests on and its date. "Transferred under our separation agreement dated March 3, 2026" says more than "personal reasons". Do not include anything you would not want an official to read; the statement is sworn and kept on file.
There are three ruled lines on the official page, so our box stops at about 270 characters. If your explanation needs more, a separate sworn statement or the document itself, such as the agreement, can go with your paperwork.
Before you book, check these
- Using this form for a family gift. Gifts between listed relatives have their own form, the 1157E.
- Several vehicles on one form. The form asks for a separate list when more than one vehicle is transferred. Ours prints one vehicle; for several, call us first.
- A different name than on the permit. Write each name exactly as it appears on the ownership and on your photo ID.
- Missing the corporate addendum. For a corporation and shareholder transfer, get the addendum from ServiceOntario and attach it after signing.
Swearing the 1155E online
The 1155E is signed by both people named in it. After Book and Sign you choose a time, add the second signer's email and both IDs, and pay. Each of you joins the call from your own device, swears the statement and signs on screen; the commissioner completes the "SWORN before me" block, and the finished form is emailed to you both.

FAQ
Can only one of us sign?
The form has signature lines for both the recipient and the donor, and both swear it. If one person cannot attend, talk to ServiceOntario about the alternatives before booking.
Is a separation agreement enough without this form?
ServiceOntario may accept a signed separation agreement in some cases. Ask them which document they want for your transfer.
What is the "information addendum"?
A ServiceOntario document required for exempt transfers between corporations, or a corporation and a shareholder. You attach it to the sworn statement.
Which edition of the form is this?
The Ministry of Finance edition marked 1155E (2014/06) at the bottom of the page.
Can we sign from different cities?
Yes. Each person joins from their own device and location, with their own photo ID.
Do you decide whether our transfer is exempt?
No. We verify identities and commission the statement. The exemption is decided by ServiceOntario under the Ministry's rules.
