Sworn Statement for the Transfer of a Used Motor Vehicle (Form 013-1155) — Commissioned Online
Ontario Ministry of Finance Form 013-1155, sworn by both the person receiving the vehicle and the person transferring it, to claim a retail sales tax exemption on certain used vehicle transfers.
Normally, when a used vehicle changes hands in Ontario, retail sales tax is paid when the new owner registers it. Some transfers are exempt, for example certain transfers between a corporation and its shareholder. To claim the exemption, both parties describe the vehicle and the reason for the transfer in this sworn statement. Gifts between family members use a different form, the Sworn Statement for a Family Gift of a Used Vehicle.
Both the recipient and the donor swear the statement. Select two signers when you book: each person joins the video call, shows their own photo ID and signs while the commissioner watches. If the transfer involves a corporation, ServiceOntario may also ask for an information addendum to be attached.

How is this different from a bill of sale?
A bill of sale records a sale for money. This form is a sworn statement used to claim a tax exemption when a vehicle is transferred without a taxable sale.
Who signs?
Both the recipient and the donor. Each person shows their own photo ID on the call.
Is this the form for a gift to a family member?
No. Family gifts use the Sworn Statement for a Family Gift of a Used Vehicle (Form 013-1157).
