When OSAP asks for this affidavit

OSAP treats a student as a sole-support parent when they are single, separated, divorced or widowed and a child lives with them during the study period. A separated or divorced student usually proves it with a separation agreement, a court order or a divorce judgment. When none of those exists, OSAP accepts a sworn affidavit instead, and students who were never married or are widowed confirm their situation the same way.

OSAP has no official form for this affidavit, so each student writes their own. Our template asks the questions in the order a financial aid officer reads them and prints numbered paragraphs that are easy to check.

What the affidavit states

ParagraphWhat it says
1That you apply to OSAP, for which study period, and at which school if you add it
2 and 3Your marital status, with the date of separation, divorce or death, and that you do not live with a spouse or partner
4Each child's name and date of birth
5 and 6That the children live with you full time or at least half the time, and the custody arrangement
7 and 8Child support and spousal support you receive per month, or that you receive none
9The purpose of the affidavit

Full time or at least half the time

OSAP looks for children who live with you at least 50% of the study period. Choose "full time" when the other parent is not involved, or "at least 50% of the time" for shared parenting. The optional custody box lets you describe the arrangement in your own words, for example the visiting schedule; if you leave it empty, the affidavit says there is no order or written agreement giving custody to the other parent.

Before you upload it

  1. Support left out. Child and spousal support are part of the assessment. Enter the monthly amount, or answer No.
  2. Widowed without the certificate. Upload a copy of your spouse's death certificate with the affidavit.
  3. A study period that does not match. Use the same start and end dates as your OSAP application.

Swearing it by video

FAQ

Is there an official OSAP form for this?

No. OSAP accepts an affidavit in your own words when you have no separation agreement, court order or divorce judgment. This template gives it a clear structure.

I share parenting with my ex. Can I still use it?

Choose "at least 50% of the time" and describe the arrangement. OSAP decides whether you qualify.

Who signs it?

You, the student parent, before a commissioner for taking affidavits.

Can I list more than four children?

Up to four fit on this template. For more, prepare your own affidavit and upload it when you book.

Do you check whether I qualify?

No. We do not give legal advice or review your answers. Ask OSAP or your financial aid office about eligibility.

Can I download the filled affidavit?

No. It is attached to your booking and sworn at your video appointment; the commissioned PDF is emailed to you afterwards.
