Student & OSAP Affidavits
OSAP Affidavit of Marital Status — Commissioned Online
A sworn statement confirming a student’s marital status, used when OSAP needs it verified.
Your marital status changes how OSAP assesses your application: whether it looks at parental or spousal income, and which category you fall into. If your status changed recently or is not clear from your records, you may be asked to confirm it under oath.
Common situations include a recent marriage, a separation, or a divorce that is not yet final. You upload the affidavit unsigned, join a short video call, show your ID and sign in front of the commissioner.
Commissioned in accordance with O. Reg. 431/20. We do not advise on which marital status applies to you; please confirm that with OSAP or your school.
