OSAP Affidavit of Sole Support Parent — Notarized Online
A declaration confirming that a student is being solely supported by one parent, sworn over a secure video call for OSAP purposes.
When only one parent is providing financial support — due to separation, single parenthood, or another family circumstance — OSAP often asks for this sworn declaration to properly assess a student's aid eligibility.
Bring the OSAP form to your appointment unsigned. The commissioner verifies your photo ID, witnesses your signature, and applies the seal.
