DS-3053 Statement of Consent — Signed Online

The U.S. form a parent signs to consent to a U.S. passport for a child under 16 when that parent cannot attend the application.

A U.S. passport for a child under 16 normally needs both parents present. When one parent cannot attend, that parent can give consent in writing on Form DS-3053, signed in front of a notary, and the other parent submits it with the application.

Many U.S. families in Ontario use this form when one parent is travelling, working away or living separately. The consenting parent joins the video call, shows their photo ID, and signs while the notary watches.

Requirements for U.S. passports are set by the U.S. Department of State. Please check with the passport office or consulate handling the application that the form will be accepted as signed.
