RIN Request Sworn Statement — Commissioned Online
The sworn statement ServiceOntario asks for when a business or organization applies for a Registrant Identification Number.
A Registrant Identification Number, or RIN, identifies a business or organization that registers vehicles in Ontario. ServiceOntario may ask for a sworn statement as part of the request.
The statement is usually signed by an owner, director or authorized person of the business. They join the video call, show their photo ID and sign while the commissioner watches, without leaving the office.

A person with authority to act for the business, as set out in the form.
Can I add a corporate document in the same call?
Yes. Add it as an additional document when booking.
Do you file it with ServiceOntario?
No. You submit the commissioned statement yourself.
