The federal Form 2 you swear when you withdraw or transfer money from a federally regulated locked-in plan, sworn over a secure video call.

Form 2 is free to download from the Office of the Superintendent of Financial Institutions (OSFI). Fill it in on your computer, leave the signatures blank, and sign it with us by video.
Form 2 comes from Schedule V of the federal Pension Benefits Standards Regulations, 1985. It is used when the owner of a federally regulated locked-in plan, such as a locked-in RRSP, a life income fund, a restricted locked-in savings plan or a restricted life income fund, applies to withdraw or transfer funds.
In the form, you state whether you have a spouse or common-law partner. If you do, your spouse or partner completes their own part, confirming they understand the consequences of the withdrawal and giving their consent.
Each person who signs swears their part before a notary public or commissioner for taking affidavits. If both of you sign, select two signers when you book: each person joins the video call and shows their own photo ID.

Where do I get Form 2?
From the OSFI website, or from your financial institution, which may send it with its own application package.
Does my spouse have to sign too?
Only if you have a spouse or common-law partner. In that case they complete and swear their own part of the form.
Is this the same as the Ontario locked-in forms?
No. Form 2 is for federally regulated plans. Plans regulated by Ontario use different forms from FSRA, so check with your financial institution which one applies to you.
Can this be signed online?
