Service Canada Separation Statement — Notarized Online
A sworn statement required by Service Canada to update marital status for government benefits, made over a secure video call.
Government benefits are often calculated partly on household status, so when a couple separates, Service Canada asks for it in writing, sworn, so there is no ambiguity about the fact or the date.
Bring the form to the call unsigned. The commissioner verifies your photo ID, witnesses your signature, and applies the seal.
