Affidavit for a Lost Cheque (FRO) — Commissioned Online

The sworn statement the Family Responsibility Office asks for before it reissues a lost or stolen support payment cheque.

Book an Appointment

When a support payment cheque from the Family Responsibility Office goes missing in the mail or is stolen, the office asks the recipient to confirm what happened in a sworn statement before issuing a replacement.

Waiting for a support payment is stressful, so the faster the affidavit is sworn, the better. Book a time, upload the unsigned form, show your photo ID on camera, and sign. The completed document is emailed to you right after the call.

Commissioned in accordance with O. Reg. 431/20. Questions about your support payments should go to the Family Responsibility Office.
