A mandatory Ontario court form in family cases involving children, sworn over a secure video call.
When parents are before the court about decision-making, parenting time, or contact, the judge needs a sworn account from each of them, and in Ontario that is what Form 35.1 provides. It has to be sworn before it can be filed.
