Lost, damaged, or never issued: plenty of people end up needing to prove where and when they were born without the certificate in hand. Pension offices, immigration authorities, and some overseas institutions will accept a sworn affidavit describing the birth details, either while a replacement is arranged or when no certificate exists at all.

The affidavit arrives unsigned. On the call, the commissioner checks your photo ID, watches you sign, and applies the seal.

Commissioned in accordance with O. Reg. 431/20. We do not verify the birth details themselves, and we cannot tell you whether a particular office will take an affidavit in place of a certificate. That is worth confirming with them first.

Will an affidavit replace my birth certificate?

Only if the office asking for it agrees to accept one. Many will, some will not, so check with them before your appointment.

Who signs the affidavit?

Typically the person named, or a relative with direct knowledge of the birth. The receiving office will tell you which it wants.

Can this be sworn online?
