A sworn statement confirming that two names belong to the same person, used after marriage, divorce or a legal name change.

After a name change, older records often still show the previous name: a bank account, a pension, a school transcript, an insurance policy or a foreign document. An Affidavit of Name Change ties the old and new names together, so an institution can update its records without doubting that you are the same person.

You upload the affidavit unsigned, together with your photo ID. On the video call the commissioner confirms your identity, administers the oath or affirmation, and witnesses your signature before commissioning the document.

Appointments are conducted in accordance with Ontario Regulation 431/20. This affidavit supports a records update; it does not replace the official ServiceOntario process for a legal name change. We do not provide legal advice on its contents.

Is this the same as applying to change my name in Ontario?

No. A legal name change is done through ServiceOntario. This affidavit confirms that your old and new names refer to the same person.

Which ID should I show on the call?

A valid government photo ID, ideally one that shows your current name.

Can I use it for an institution outside Canada?

Many foreign institutions accept it. Requirements vary, so check with the receiving office first.
