CCDC 9A is completed by a general contractor to declare that subcontractors, labourers, and suppliers have been paid from a progress payment before the next one is released. It is a routine part of holdback and lien-protection procedures on construction projects across Ontario.

Bring the completed CCDC 9A form to your appointment unsigned. The commissioner verifies your photo ID and witnesses you sign as the authorized representative of the contracting company, then applies the seal.

Notarized in accordance with O. Reg. 431/20. We do not review the payment figures on the form or advise on construction lien matters — our role is to verify identity and witness the declaration.
