Form 13 Financial Statement (Support Claims) Sworn Online in Ontario
In an Ontario family case about child support or spousal support, each side has to swear to their finances on Form 13. You can swear or affirm yours with us over a secure video call.
Get the Official Form
Find Form 13 in the Family Law Rules list on Ontario Court Forms, where the newest version is always posted, as a PDF or a fillable Word file. Complete it on your computer, leave the signature and the commissioner section empty, and swear it with us by video.
Form 13 is needed whenever support is part of a family case and property is not. Typical situations are a parent asking for child support, a former spouse asking for spousal support, or either of them bringing a motion to change an existing support order. When property is also being divided, the longer Form 13.1 is used instead.
If the court later asks you for an updated financial statement, or for an affidavit confirming that your earlier statement is still accurate, that document can be sworn with us the same way.
When the form is ready, upload it unsigned with your photo ID and book a time. On the call you confirm under oath or affirmation that it is true and complete, you sign, and the commissioner completes the "sworn before me" section and seals it. The commissioned PDF is emailed to you for serving and filing.
No legal advice, no drafting. We act only as commissioners, under O. Reg. 431/20: we confirm your identity and take your oath or affirmation. We do not fill in, prepare or review the form, choose between Form 13 and Form 13.1, or advise on support. For that, speak with a lawyer or the family law information centre at the courthouse.

Form 13: Financial Statement (Support Claims) FAQ
What is the difference between Form 13 and Form 13.1?
Form 13 is for cases about support only. Form 13.1 is for cases that also include a claim about property. The court office or your lawyer can tell you which one applies.
Can I swear or affirm it online?
Yes. Remote commissioning by video is allowed in Ontario, and you can join from anywhere with your photo ID.
Do you help fill in Form 13?
No. We do not give legal advice and we do not draft, complete or check the form. You prepare it yourself or with your lawyer, and we commission it.
Are attachments commissioned too?
Only documents your statement refers to as exhibits. Each one is marked and signed by the commissioner on the same call, as an additional document.
