FRO VAPS Proposal: Your Financial Statement (Form 4) Sworn Online

A VAPS Proposal to the Family Responsibility Office must come with a sworn Financial Statement (Form 4). We commission the financial statement over a secure video call.

Get the Official Form

The Financial Statement (Form 4, FRO-010) and the VAPS Proposal (FRO-004) are both free on the Government of Ontario forms site. They are fillable PDFs that open best in Adobe Acrobat Reader. Fill in the financial statement completely, leave the signature and the commissioner section blank, and sign it with us by video.

When a parent or former spouse has fallen behind on child support or spousal support, the Family Responsibility Office (FRO) lets them propose a payment plan for the arrears, called a Voluntary Arrears Payment Schedule (VAPS). In the VAPS Proposal you set out your case number and the payment plan you are proposing for the arrears. FRO reviews it and decides whether to accept it.

The VAPS Proposal itself is simply signed by the payor. The part that has to be sworn is the Financial Statement (Form 4) that goes with it. The proposal includes a confirmation that your sworn financial statement and proof of income are attached, and the financial statement has a "Sworn before me" section that must be completed by a commissioner for taking affidavits or a notary public.

Fill in all parts of the financial statement before the appointment, including your employment, income, expenses, assets and debts, and leave the signature empty. On the video call we verify your photo ID, you swear or affirm that the contents are true and sign, and we complete the commissioner section and seal it. The commissioned statement is emailed to you, ready to send to FRO together with your VAPS Proposal and proof of income.

Commissioned in accordance with O. Reg. 431/20. We do not help fill in the figures, choose the payment amounts, contact FRO for you or give legal advice about support. Questions about your case or arrears should go to the Family Responsibility Office.

Both are free on the Government of Ontario forms site: the Financial Statement as FRO-010 and the VAPS Proposal as FRO-004. Use the link on this page for the financial statement.

Can I swear the financial statement online?

Yes. Upload the completed but unsigned statement and your photo ID when you book, then swear or affirm and sign live on a secure video call.

Should I sign the financial statement before the appointment?

No. Fill in everything except the signature. You sign during the call, in front of the commissioner.

Do you send the documents to FRO for me?
