A sworn statement of a married or common-law student’s spouse’s income, for OSAP assessment.

If you are married or in a common-law relationship, OSAP considers your spouse’s income too. When it cannot be confirmed through tax records, your spouse may need to declare it in a sworn affidavit.

Your spouse joins the video call, shows their photo ID and signs while the commissioner watches. If you both need to sign documents for your application, you can book them in the same appointment.

Commissioned in accordance with O. Reg. 431/20. Questions about how spousal income affects your funding are best answered by your school’s financial aid office.

Who signs this affidavit, me or my spouse?

The person whose income is being declared, usually your spouse, as set out in the form.

Can we do two OSAP documents in one appointment?

Yes. Add the second document when booking; each additional document is billed separately.

Is the commissioned copy sent by email?

Yes, shortly after the video call.
